  • If you need to add your SSN to your student record, please submit the update here.

  STEPS TO ACCESS YOUR ADMISSIONS FORMS

  1. Begin by logging in to your Applicant Portal.
  2. You will be taken to your Admissions Checklist. Select “Forms“ on your checklist.
  3. All of your forms are accessible from this screen and you can fill them all out here.

  Please Note: Any update(s) to your Application Checklist will not be reflected on your Applicant Portal until your form is processed by the Office of Admissions. Please allow up to 4-6 weeks from the time the form is received for processing.
